As a Patient Centered Medical Home, we are constantly working to provide the highest quality care to our patients. In 2014 we began several projects to increase the number of our patients who were up to date in important cancer screening tests. We have added additional providers to care for our increasing number of patients, run monthly reports and reached out to patients who were overdue for an important test with postcards provided by the American Cancer Society, and phone calls. The results are in:

*A 19% increase in the number of patients who had received their mammogram in the past year.

*A 35% increase in patients up to date on their pap smears.

*A 25% increase in patients who completed their colon cancer screening test.

Watch this space for future updates on our efforts to help our patients achieve their best health.
