Our goal is to develop and maintain a long-term, healing partnership with each of our patients, while ensuring the best possible health outcomes. Serving communities in the Yolo County area, CommuniCare provides comprehensive health care services delivered by a dedicated team of providers and support staff through our clinic sites and outreach programs.

CommuniCare provides health services for one in every nine residents in the Yolo County area. Our services include primary medical and dental health care, behavioral health services, substance abuse treatment, health education and support services. We strive to deliver services in a supportive, safe, non-judgmental manner that stresses patient involvement and responsibility, health education, counseling and preventive health care in all our programs.
