NOTICE: Effective Monday, August 15, 2016, the hours for CODAC at Alvernon (630 N. Alvernon) and CODAC at Cobblestone Court (1075 E. Ft. Lowell) will be 7:00 a.m. to 6:00 p.m.

Not only does CODAC provide a full continuum of professional behavioral health care to adults and youth (ages 6+), but we also provide primary care. Primary care providers, psychiatrists, nurses, medical assistants, therapists, case managers, peer support specialists and other staff work together as a single team to treat the whole you, all in in one convenient location. This makes it easier for you to get all the care you need.
