Community Health Development, Inc. (CHDI) is in the process of conducting a Community Needs Assessment in preparation for the development of our Strategic Plan for the next three years. The Community Needs Assessment is a process for determining needs or "gaps" between current conditions and desired conditions or "wants". The information collected through the needs assessment process forms part of the foundation for the CHDI Board of Directors and staff to develop a Strategic Plan that outlines the agency’s priorities for the next three years. We want the goals of our Strategic Plan to accurately address what the community needs. To this end, your input is very important to us.
