In 2010, it was estimated that 191,235 Lee County residents were living in low-income households, with more than 130,000 of them lacking access to primary health care services. In response to this acute community need, Lee Memorial Health System opened primary care clinics in the Dunbar and North Fort Myers communities during 2011 and later in Cape Coral. These locations provided more than 10,829 office visits to 4,165 uninsured and economically distressed residents of Lee County during 2013.

In an effort to provide the highest quality of care possible patient care teams have been assembled consistent with a patient-centered medical home approach to providing primary care. Patient care teams are formed from clinical staff that includes physicians, advanced registered nurse practitioners, nurses, patient registration specialists, case managers, information referral specialists, chronic disease counselors and a nutritionist.

Traditionally, community health centers and clinics have had to focus on moving the most patients through the “care process” as possible. However, at Lee Community Healthcare, we believe that time taken to listen to and empathize with our patients is time well spent. While some models of care call for 15 and 30 minute long appointments, Lee Community Healthcare has chosen to allow longer times for scheduled appointments in an effort to improve the quality of care provided and the bond between us and our patients.

The Right Care starts with a commitment by our care teams to focus on patients and what is best for them. It strives to develop a close provider-patient relationship through longer visits, and seeks to empower each patient to truly understand and take ownership in their own health.

The Right Place means placing offices in those Lee County communities where primary care services for the uninsured and poor are lacking and maintaining a 15-minute drive time from these locations to local emergency rooms.

The Right Time involves providing extended hours during the workweek and on the weekend so that working families can more easily access the care they need. Emphasizing preventive patient care before a serious health issue arises means a healthier patient and a reduced likelihood of them having to go to the hospital.

Based on household size and family income Lee Community Healthcare patients can qualify for discounts on their care. For example, a mother or father with two children earning less than $23,850 can be seen by an Lee Community Healthcare provider for as little as $15.00. We welcome all existing Medicaid and Medicare recipients as well as patients possessing private insurance.

In addition, our highly trained staff routinely assists patients in evaluating their eligibility to purchase health insurance through the Affordable Care Act health insurance exchanges, and in signing up for Medicaid or Medicare benefits. No one is ever turned away because they lack the money to pay for their care.
