East Arkansas Family Health Center (EAFHC) is devoted to providing accessible, affordable, and quality primary care, dental care, and support services to the underserved populations in Crittenden, Poinsett, Mississippi, and Phillips County. Within these counties, we have six primary care medical offices and an HIV/AIDS care center.

Accredited by the Joint Commission, EAFHC has developed a reputation of excellence and is considered one of the premier organizations providing patient-centered medical homes to people in eastern Arkansas who would otherwise be without access to quality primary health care services.We treat patients of all ages who are insured, uninsured and traditionally underserved. Although we are not a free health center, we do offer services at a discounted price for those who qualify based on their income and household size. We also provide transportation to and from each location, and for patients who need additional services not performed at the clinic, such as mammograms.

Not only do we diagnose and treat acute illnesses and injuries such as the flu and minor fractures, but we also treat and manage chronic diseases such as diabetes, hypertension, and obesity. Additionally, we provide OB/GYN services for girls and women and pediatric care for newborns to adolescents. In addition to primary care and dental care, EAFHC also offers diagnostic services such as blood draws and x-rays at each of our locations, as well as a full-service pharmacy at our West Memphis and Lepanto offices. EAFHC has an onsite one-day Cardiologist from Southern Cardiology at our West Memphis office. When you want comprehensive, compassionate care for you and your family, visit East Arkansas Family Health Center. Patient Centered Medical Home (PCMH) is an approach to providing total health care and putting you, the patient, at the center of your health care. Under the PCMH model of care, you will join a team that includes your provider of choice, a care team, trusted family and friends (if you wish), and most importantly, you. With this new relationship, you will be informed and involved in your health care decisions and bring together you, your family, and your health care team to make the best choices about your health.

Over the next several months, East Arkansas Family Health Center will be preparing to receive recognition as a PCMH at all of our five medical facilities through the National Committee for Quality Assurance (NCQA). You may notice some changes in the way we provide care but the health and wellness of our patients will remain our primary priority.
