Modeled after Bonaventure House, The Harbor opened its doors in 1998 as a transitional living facility in Waukegan, IL. With eight beds, The Harbor provides housing, supportive services and spiritual care, helping individuals reach independent living.

North of Chicago, the Lake County AIDS Housing Project began looking for a site to establish a transitional living facility for people with AIDS in the mid-1990s. Citing a study conducted by the Chicago-based Alexian Brothers Health Services, nearly two-thirds of the people living with AIDS in Lake County also suffered from problems with substance abuse.

Despite local prejudice and protests, The Alexian Brothers once again forged ahead to establish The Harbor: a renovated, 122-year-old Georgian-style house in Waukegan, Illinois. Opening its doors in 1998, the house would serve as a transitional living facility to accommodate eight adults living with AIDS.

Modeled after Bonaventure House, The Harbor provides a structured and supportive living environment with 24/7 staffing, single furnished private rooms, a family room, physical fitness equipment, a learning center, and more.

Linked to established professional resources in the community such as Catholic Charities, Lake County Health Department/Community Health Center, Capt. James A. Lovell Federal Health Care Center, and NICASA (Northern Illinois Council Against Substance Abuse), The Harbor provides off-site services for both primary medical care and HIV/AIDS case management. The Harbor manages our Transitional Living Program and is the only licensed recovery home for people with HIV/AIDS in Lake County.

Through our Transitional Living Program, Bonaventure House offers our residents a wide array of supportive services, including:

CASE MANAGEMENT Each resident develops a comprehensive individualized service plan with an on-site Case Manager/Recovery Coordinator (CM/RC). Residents meet with their case managers each week to monitor and evaluate progress. Case management and recovery services are vital in preparing residents to become self-sufficient and return toindependent living in the community.

RECOVERY The Harbor is a licensed recovery home through the state of Illinois. Its residents maintain a safe, sobriety-based living facility with the help of one-on-one and group services, peer support and recovery support provided by licensed CADC’s.

OCCUPATIONAL THERAPY Group and individual sessions help residents to identify strategies needed to overcome barriers and work towards healthy, independent lives.

SPIRITUAL CARE Our program is holistic, which includes promoting spiritual well-being alongside physical, mental, and emotional well-being. All spiritual services are optional and ecumenical.

AFTER CAREAfter Care provides continuing support to graduates of the Transitional Living Program to achieve stable, independent living.

Applicants for The Harbor's Transitional Living Program must:

1. Have a diagnosis of HIV or AIDS;

2. Be homeless or at-risk of homelessness;

3. Be at least 18 years old; and

4. If in recovery, have 30 days of substantiated sober time, and be committed to remaining sober
