The Southern Hills Case Management Program provides goal-oriented, individualized, systematic support of the persons served through ongoing assessment, planning, advocacy, monitoring activities, coordination, and linkage with other program areas and community resources.

Our medical services are unlike those provided by your primary care (family) physician and our prescribers will not be prescribing medication for your non-psychiatric medical conditions. We have nurses who work with our prescribers, just as in any doctor’s office. Since the nurse’s service is essential to quality care, it is included in our medical service program. More

Southern Hills provides substance abuse services to adolescents, adults, and seniors. Services are provided to clients based on an individual treatment plan, which is developed in conjunction with the client after a thorough assessment. When a client presents for treatment, he/she is interviewed by a therapist who completes an evaluative interview, initial assessment, substance use history and medical information summary. More

Southern Hills was established in 1968 and serves the residents of Dubois, Perry, Spencer, Orange, and Crawford Counties in southwestern Indiana.

The Center employs over 130 full and part-time staff members at its five county offices and four residential facilities.

Services are provided in Jasper (Dubois County), English (Crawford County), Paoli (Orange County), Tell City (Perry County), and Rockport and Dale (Spencer County). More
