We are a community health center. By providing primary care and counseling on site and coordinating a variety of support services, our goal is your complete wellness. If you are uninsured, our team can help you determine what insurance options are right for you. By accepting Healthy Michigan, Medicaid, Medicare, private insurance, and offering an income-based sliding fee scale for the uninsured, we make healthcare accessible to every person in your family.

Take Note: Beginning Monday, October 24th, 2016, we will be moving to 1719 S. Garfield, Traverse City, MI 49686! We’ll have the same phone number, same providers and staff, same services and a lab, a BATA stop our front and ample parking.

We treat our patients like people, not symptoms. Our doctors, nurse practitioners, and staff are with us because they are genuinely passionate about community health. From routine check-ups, diagnosing and treating illness, vaccinations, screening tests, and more, we take a proactive, personal approach to healthcare.

Our goal is your complete wellness. For patients who have established care with one of our providers, we offer a variety of support services such as on-site counseling, translation services, transportation assistance and help accessing dental care, if needed.
