Contribute to the health of our community—that’s our mission. Our outreach professionals meet you where you are, at schools, stores, other organizations, to talk about health education, and enrollment. Our community health worker provides health-related support for diabetics in their own home.

Our welcome wagon, these professionals educate new patients about all the support services we offer. They can even assist with insurance enrollment.

Our Navigators are certified by the state of Indiana to provide free education and assistance for insurance enrollment. Whether you're a Heart City patient or not, we want you to be an active partner in your health, and that begins with health coverage.

We have a full retail pharmacy on-site at our Simpson location. As a patient, you can get the most prescriptions at a discounted price. We can also assist you in registering for Medicine Assistance Programs (MAP). If you qualify, the MAP can help you receive prescriptions for a fraction of the original cost.

Our financial services staff helps you determine the best method of payment for the services received, including a sliding fee scale that can help you save up to 90% on the cost of services based on your household income and family size.

Patient Care Coordinators are trained professionals who provide education, training and assistance managing chronic health conditions such as diabetes, obesity or hypertension. A valued part of your patient care team, these coordinators supplement the care provided by your physician and help ensure a healthier community.
