Sebasticook Family Doctors

SFD is a Federally Qualified Health Center that offers medical services, dental services, behavioral health services, a sliding-fee scale for payment that is based on what the patient can afford, several prescription assistance programs, and much more. 

SFD has a 24 hour/7 day per week FoneMed system which provides our patients with access to a nurse and provider who offer clinical advice when our offices are closed. This system is designed to best meet the needs of our patients, even in the middle of the night. The FoneMed system can be reached by calling any of our offices and following the automated prompts.
