Following a hospital stay related to illness, injury, or surgery, many patients require further care to reach an optimal level of strength and wellness before they can safely return to their home. The Swing Bed Program at Minnie Hamilton Health System helps patients achieve this goal through hospital-based nursing care and rehabilitation services.

Minnie Hamilton Health System offers a swing bed program allowing patients to rehabilitate in our facility for an extended length of time while remaining close to home. Referrals for MHHS’s swing bed program do not require a nursing home PAS approval process. We provide continued recovery and rehabilitation services designed specifically for each patient’s needs. Examples of patients entering our swing bed program include:

• Care and rehabilitation services after a stroke

• Care and therapy services after orthopedic surgery such as joint replacement

• IV therapy to recover from an illness.

Our swing bed patients receive around-the-clock care from our nursing staff. Depending upon the individual care plan, our swing bed patients receive:

• Physical, Occupational and Speech Therapy

• End of Life Care

Patients can often be taken care of by their primary care provider if they are established with MHHS clinic services. In general, the average length of stay in the program is 2-3 weeks with the goal to transition back to home.

The Community Needs Assessment has multiple purposes. It's first purpose is to meet the requirements of the Patient Protection and Affordable Care Act (“ACA”), Public Law 111-148 enacted section 501(r) of the Code, which imposes additional requirements on charitable hospital organizations. Section 501(r)(3) requires a hospital organization to conduct a comprehensive Community Health Needs Assessment (“CHNA”) at least once every three years and to make the CHNA widely available by placing it on the hospital’s web site.

It's other purpose is to provide the hospital with a document that highlights the data findings, outlines key needs in the hospital’s service area, and provides direction and opportunities for action through the adoption of an implementation strategy to meet those needs. This CHNA looks at health status, barriers to care, and other demographic and social issues affecting people and organizations throughout Calhoun and Gilmer counties and is intended to help improve the health of the area’s residents.
