Welcome to Jordan Health, a system of comprehensive health centers and primary care offices serving nearly 30,000 patients in the Cities of Rochester and Canandaigua, New York.

What is today Jordan Health began as individual centers established to meet the demand of Rochester residents for neighborhood-based healthcare that was affordable and accessible to all.

Today we are no longer one health center but a network spanning Monroe and Ontario counties. Jordan Health’s Comprehensive Health Centers are designated as a Level III Patient-Centered Medical Homes (PCMH) by the National Committee on Quality Assurance and all of our sites are accredited by the Joint Commission.

Jordan Health has begun the journey to transform our patient care model from a provider-centric model to a team care model. In transforming our practice, we have built comprehensive teams that include not only providers and nurses, but also the patient and their family. In this team care environment, the entire group supports the patient by understanding their current health status and setting goals to achieve a healthier lifestyle. This is accomplished through strong relationships, enhanced listening skills and active partnership with the patient. Ultimately, our teams will extend beyond the clinical visit to include care managers, behavioral health providers and community partners. This high-quality, comprehensive community-based care is what defines the Jordan Health experience.

What is today Jordan Health began more than 100 years ago as a neighborhood-based health service to immigrant women and their infant children. As the need for accessible healthcare grew, our community responded. Through the determination of our residents and the work of Dr. Anthony L. Jordan and Dr. Kenneth Woodward, Rochester became a national leader in the delivery of affordable accessible healthcare to the underserved, and established the Anthony L. Jordan, Brown Square and Woodward Health Centers.

Given our changing economy, aging population and growing refugee communities, the demand for these health services continues to grow. We have met this need by opening seven additional healthcare offices in more neighborhoods and underserved communities.

Our challenge today is to integrate and evolve to remain leaders in community-based primary care. To do this, we are transforming our patient care model and focusing on the patient experience. We are no longer individual health centers but a network of ten unique, comprehensive health centers and primary care offices that facilitate the delivery of primary care to nearly 30,000 patients in two counties. We are Jordan Health.
