The mission of Canyonlands Healthcare is to promote healthier lives through affordable, accessible, integrated primary care.

Canyonlands Healthcare offers comprehensive one-stop services and a health care home for patients in each of the communities served by the organization. Our Health Centers provide more than just treatment for illness or episodic conditions. We provide ongoing care and health management for families and individuals throughout their lives.

Canyonlands Healthcare is a private, not for profit, corporation providing consumer-directed health centers which provide high quality, cost-effective and comprehensive primary and preventive care. Canyonlands offers special programs designed to improve access to care for those who face financial barriers and need assistance. Canyonlands Healthcare has been designated as a Federally Qualified Health Center (FQHC) and as such is supported by federal grants under the U.S. Public Health Service Act.

Canyonlands Healthcare is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). As such, malpractice insurance coverage is provided through the Federal Tort Claims Act (FTCA) for all Canyonlands providers and locations.

Welcome to Canyonlands Healthcare. Our team here at Canyonlands would like to introduce you to all of the services offered at our 10 locations in Northern and Southeast Arizona. We offer primary medical care, integrated behavioral health services, dental care, family planning and reproductive health services, women's health, lab, discounted pharmacy services, black lung screening, vaccines for adults and children, financial assistance, enrollment assistance with Arizona Medicaid (AHCCCS) and Marketplace programs, care management, and the list goes on. Canyonlands is your health home that offers one stop for comprehensive services.

Canyonlands began in 1973 with the founding of Lake Powell Medical Center in Page, Arizona. Since that time, the organization has grown to include numerous locations and expanded services. My team and I are committed to providing quality care with dignity and compassion. Our goal is to provide affordable health care to all individuals in a comfortable, safe, culturally sensitive environment.

Canyonlands strives to recruit highly skilled medical providers, dentists, behavioral health specialists, pharmacists, and support staff. We treat all patients with the same care and respect we expect for our own families.

Canyonlands Healthcare is your community health center. We care about you, your family and the overall health and well-being of our communities. If you are currently a patient, I thank you for choosing Canyonlands to care for you and your family. If you are looking for qualified, skilled, and compassionate medical, behavioral, and/or dental providers, I invite you to select Canyonlands Healthcare as your health care home. Our array of services is designed to be easily accessible to all, regardless of insurance status or ability to pay. We are committed to be there when you need care.

Thank you for your interest in Canyonlands Healthcare,
