Utah Partners for Health provides compassionate, comprehensive, quality care to empower underserved individuals, families, and communities in a patient centered medical home.

Utah Partners for Health collaborates with multiple for-profit clinics willing to provide in-kind donations to our clients. This program serves Magna, West Valley, and Kearns Utah. Services include works with Medical Care, Preventive Dental, Eye Care, Physical Therapy, Radiology, Ultrasound and Counseling.

MVHC is a Federally Qualified Health Center (FQHC), located in Midvale, Utah providing primary health care services to all regardless of the ability to pay. We operate under a discounted sliding fee schedule based on proof of income. We accept Medicaid, Medicare, PCN, CHIP and most other insurances.

We are a patient centered family medical home, which means our providers can see patients of ALL AGES and we love for you to bring your whole family in so we can get to know everyone! We provide primary care services, but are also linked with many specialist and ancillary services so we can help you get the best prices for low- or no-cost specialty care. All staff are bilingual (English, Spanish, Portuguese and Tongan) and translation services are available if needed. An after-hours nurse triage line is available for medical concerns when the office is closed – This line does not schedule appointments for the clinic.

We collaborate with 24 different community based organizations providing access to healthcare to low-income and underserved individuals who experience cultural barriers in accessing care.

If you are uninsured and in need of primary care, please call 385-204-6257 to find out about our mobile clinics and schedule your free appointment. Our mobile clinic is out three times a week hosting free primary care visits in Salt Lake and Tooele Counties. We also partner with Charity Vision, a mobile eye care clinic that is out twice a week for free vision exams, prescriptions and free or reduced cost glasses.
