Bear Lake Community Health Centers, “Your Medical Home” provider, provides excellent Family Practice services from all of our Providers which include, Family Practice, OB/Gyn Services, Physicians, Nurse Practitioners and Physician’s Assistants who are all able to provide treatment to infants, children, adults and senior citizens.

Our health centers offer a large variety of medications at discounted rates for your convenience. Visit or call our pharmacist to see how we can decrease your pharmacy costs and better improve your health.

Bear Lake Community Health Centers is “Your Medical Home” provider that offers dental healthcare from dentists and hygienists eager and excited to serve you, who are specialized in dental healthcare.

Cache Valley Community Health Center provides a full range of OB/Gyn services to meet the health needs of the women in Cache Valley area.

Bear Lake Community Health Centers is “Your Medical Home” provider that offers exceptional behavioral health services. Our behavioral health providers offer a variety of therapy sessions in many forms to meet our patients’ diverse needs.

As a ACA organization, one of our counselors will help you understand, apply, and enroll  for health coverage through the Marketplace.
