Team-centered. Your HealthFirst team is led by your Primary Care Provider (PCP) and may include nurses, medical assistants, counselors, and/or nutritionists, as well as specialists from other agencies or providers.

To provide high quality primary healthcare, treatment, prevention and education services to the residents of our service area, regardless of their ability to pay or insurance status, and based upon available resources. HealthFirst Family Care Center, Inc. cooperates with other community and regional healthcare providers to assure our clients the fullest possible range of healthcare services.

HealthFirst was created to make quality and compassionate primary healthcare readily available to area residents. We offer complete, coordinated and individualized healthcare to people of all ages; from babies to great-grandparents.

We recognize that our clients are whole human beings with mind, body and social needs. By integrating primary and behavioral healthcare services, we are able to work with you, listen, assess your needs, and develop a specific plan to meet them.

You don't need health insurance to get care at HealthFirst and no one is ever turned away for lack of funds. Client Navigators are on staff and available to help review your current status and options under the Affordable Care Act, Medicaid, NH Health Protection Program, and/or HealthFirst Sliding Fee Discount Program.

A Patient Centered Medical Home is the primary place you go for healthcare services where you are viewed as a whole person. A place where people know and remember you, and keep track of what's going on in your life.
